The complaint has to be submitted with an affidavit with full signature / proper address quoting PIN/contact number of the complainant at the present address of the Commission.
A complaint is required to be filed giving the full name and address of the complainant and supported by an affidavit. The mobile number and e-mail id, if any, should also be provided.
Such complaint can be sent by Post or filed in the office of the Commission either personally or through his authorized agent.

2. Visit the enrolment centre; all beneficiaries must be physically present at the enrollment centre.
3. The Verifying Officer scrutinizes all documents and verifies eligibility.
4. After verification (and payment of challan when applicable), the Operator takes a photograph of and also capture the finger prints of each beneficiary.
5. The operator issues the unique AAA ID card along with the scheme handout to beneficiaries.
